Inquiry Call
I offer a FREE fifteen (15) minute Inquiry Call where you have an opportunity to ask questions and we can discuss the general suitability of my work.
Format
Individual Therapy and Individual Yoga are available in-person or virtually through a HIPAA secure, online platform, while all other services are facilitated virtually.
Insurance
I am in-network with BCBSIL (Choice, PPO) and United Healthcare (UHC) for therapeutic services. Those who prefer self pay or are uninsured are also welcomed.​
Professional Fee
My hourly fee for professional services is $140.00, unless otherwise indicated. You are welcome to request a complete list of practice standard fees.​
Payment of Fees
You are responsible for full payment of fees. Payment is accepted through electronic means via TherapyNotes, the electronic health records (EHR) program used by my practice.​
Paperwork Privacy
Information provided by you is held digitally on password protected/encrypted devices and servers. It will only be used in relation to your use of services with me at my practice.​
Solicitations
According to the the NASW Code of Ethics (4.07 Solicitations) it is unethical for a social worker to engage in solicitation of testimonial endorsements from current clients or from other people.
Conflict of Interest
According to the NASW Code of Ethics (1.06c Conflict of Interest), it is unethical for social workers to engage in dual or multiple relationships with clients or former clients.​
Ethical Conduct
You may report unethical practices of social work to the Illinois Department of Financial & Professional Regulation (IDFPR) or of yoga to Yoga Alliance.
